Payment Options

At Utah Spine and Sport we strive to not only get superior results but also make it as affordable as possible. We have competitive pricing for those paying cash, including huge money saving Passes for those who want or need ongoing care, we accept all auto insurances and most major medical insurances.

We can also work with you individually to create custom treatment or payment plans to assure you can afford the care that you need and deserve.

Self Pay: we offer competitive prices, punch passes, monthly passes and more.

Auto Insurance: we accept ALL auto insurances. Utah has a state minimum of $3,000 PIP (Personal Injury Protection) for accident victims to get care. Insurance benefits cover chiropractic, massage, acupuncture and other therapies without any copay, which means zero out of pocket expenses for care.

Major Medical Insurance:

List of major insurances we are In-Network with: Blue Cross Blue Shield, Regence, Anthem, United, UMR, Aetna, DMBA, EMI, PEHP, etc.

Out-of-Network Insurances: We will provide you with an invoice showing the codes needed for reimbursement from your insurance company. You pay up front and then can reach out to your insurance company to be reimbursed for qualifying treatment.

We typically do not work with Worker's Comp, Medicare or Medicaid. If this pertains to you then you can still pay out of pocket for your care or we can try to find another provider to help you.
